Assessment Process

  • Step 1: Service Booking

    With your permission, our Admin team, will collect your information to start the intake process. We ask for a $100 booking fee which either comes off the final fee or is refunded to you after the completion of the assessment. You will be sent links to complete an online intake form.

    Admin will also send you our service provision agreement which contains lots of information about your and our responsibility in providing quality services to you. We ask you to then forward any reports or documents important for the assessment. We will also request your NDIS Plan details (if funded by by the NDIS) to enable a formal service agreement to be made and signed. Please have prepared your NDIS Number, Plan Dates and Plan Management Company details (if appropriate).

    Admin will also arrange a time for the clinician to call you to introduce themselves and gain an understanding of who you are to ensure they adequately prepare and select assessment activities to meet client need.

  • Step 2: Service Agreement

    Our Service Agreement is completed by DocUSign and needs to be comletedand returned along with all other documentation atleast 7 das prior to your assessment

  • Step 3: Introduction Phone Call

    The clinician who will be conducting the assessment will call you at the agreed upon time to introduce themselves, ask client specific questions and explain what to expect at the assessment. This enables the clinician to customise the assessment to your needs.

  • Step 4: 3 Assessment Sessions

    You then complete 3 assessment sessions where the clinician will analyse skills and determine the root cause of the challenges. The clinician will debrief with you after each session to explain what was assessed and what the intentions or next session are.

  • Step 5: Family Collaboration Session

    Outcomes and recommendations are explained in the 4th session. Children are asked to not attend this session, as this session focuses on explaining the assessment and the next steps.

  • Step 6: Reporting

    The assessment is formalised into a report detailing outcomes and recommendations.
